Inactivate a Provider Association

To inactivate a provider on the claimant’s provider association list, select a provider on the Provider Associations page and then select the Inactivate button.

The Update Association Information dialog then displays. You can edit only the Start Date, End Date, and Comment fields on this dialog. Make any changes as needed and select OK to exit and return to the Provider Associations page where the provider no longer appears in the association list.
