Submit Enrollment Application for Review
Use this link to complete and submit the enrollment application for the enrolling Provider. Once the enrollment has been submitted, you can make no further changes to the enrollment application.
Note: You must complete all previous steps before you can complete the following procedure.
To submit the enrollment application for review:

Select the Step 14: Submit Enrollment Application for Review hyperlink.

Follow the instructions indicated on the Final Submission page for submitting signatures and supporting documentation.

Select the Submit Enrollment button.
The System submits the enrollment application for review.
