Add Locations

Use this link to add the physical and mailing address for the enrolling Provider.

To add Locations:

  1. Select the Step 2: Add Location hyperlink.

The System displays the Locations List page.

  1. Select the Add button.

The System displays the Add Provider Location page.

  1. Enter the required information marked by an asterisk (*).

  2. Select the Next button.

The System displays the Location Address page.

  1. Select the Address button.

The System displays the Address details page.

  1. Enter the appropriate address in the Address Line 1 field.

  2. Enter the appropriate Zip Code.

  3. Select the Validate Address button.

The System auto-populates the remaining fields with the appropriate information.

  1. Confirm that the auto-populated information is correct and select the OK button.

The System saves the address details, displays the Location Address page, and auto-populates the location details.

  1. Select the Next button.

The System displays the Location Address page.

Note: In addition to adding a Physical Address, you must also add a Mailing Address. If the mailing address is the same as the physical address, select the Same as Physical Address radio button or enter the address manually as described in the previous steps.

  1. Select the OK button.

The System saves the Location Address information and displays the Locations List page with any locations that were added.

  1. Select the Close button.

The System displays the Locations List page.

  1. Select the Close button.

The System displays the Enroll Provider -Facility/Agency/Organization/Institution page.
