Add Locations

Use this link to add the physical and mailing address for the enrolling Provider.

Important Note: If you chose to populate data from PECOS when starting the application, you must verify the information is correct and complete in this enrollment step. You do this by selecting an existing record link and editing the pre-populated information, if needed. You can also add and delete a record.

To add location information:

  1. Select the Add Location hyperlink.
    • If you chose to pull in data from PECOS, you will see the Location Information page. Select the Contact Name, Physical Address, and Mailing Address you want to associate to the provider. If the correct location is not among the PECOS options, you can skip entering these fields (leaving them with the "Select" options) and then manually enter this information on the next page after you select Submit. Select Submit to navigate to the Location Detail page where you can review the location information you selected or manually enter the new location information.
    • If you chose not to pull in data from PECOS, the System displays the Locations List page. Continue with the steps below.
  1. Select Add.

    The System displays the Add Provider Location page.

  2. Enter the required information marked by an asterisk (*).
  3. Select Next.

    The System displays the Location Address page.

  4. Select Address.

    The System displays the Address Details page.

  5. Enter the appropriate address in the Address Line 1 field.
  6. Enter the appropriate Zip Code.
  7. Select Validate Address.

    The System auto-populates the remaining fields with the appropriate information.

  8. Confirm that the auto-populated information is correct and select OK.

    The System saves the address details, displays the Location Address page, and auto-populates the location details.

  9. Select Next.

    The System displays the Location Address page.

    Note: In addition to adding a Physical Address, you must also add a Mailing Address. If the mailing address is the same as the physical address, select the Same as Physical Address radio button or enter the address manually as described in the previous steps.

  10. Select OK.

    The System saves the Location Address information and displays the Locations List page with any locations that were added.

  11. Select Close.

    The System displays the Enroll Provider page for the provider type.
