Add Locations

Use this link to add the contact information, physical address, and mailing address for the enrolling Provider.

  1. Select the Add Location hyperlink.
  1. Select Add.

    The System displays the Location Information page.

  2. Enter the required information marked by an asterisk (*).
  3. Select Submit.

    The System saves the Location Contact and Address information and displays the Locations List page with the location that was added.

  4. Select Close.

    The System displays the Enroll Provider page for the provider type.
