Add Licenses and Certifications

Use this link to add license/certifications required for the enrolling Provider. Authorized users Provider enrollment worker can may click on the required credentials link button to check if the license/certification is required. the required credentials and the system will showto reveal the required license or certification is required which is dynamic and will change based on Provider type selection on the Bbasic Iinformation page. The SSystem uses this information to approve or deny the request, and the expiration dates trigger recertification requirements and notifications.

Note: You must complete all previous steps before you can complete the following procedure

To add licenses and certifications:

  1. Select the Step 5: Add Licenses and Certifications hyperlink.

  2. Select the Add button.

The System displays the Add License/Certification page.

  1. Select a Certification or License from the available radio buttons.

Note: If you choose the N-License or Certification not required then you will be required to provide evidence that a license or certification is not required.

  1. Enter the required information marked by an asterisk (*).

  2. Select the OK button

The System saves the License/Certification and displays an updated License/Certification List page.

  1. Select the Close button.

The System displays the Enroll Provider -Group Practice page.
