Add Identifiers

Use this link to add identifiers for the enrolling Provider. These identifiers are maintained by external entities and uniquely identify the Provider. (For example, National Provider Identifier (NPI)) This information is used required to approve or deny the enrollment request.

Important Note: If you chose to populate data from PECOS when starting the application, you must verify the information is correct and complete in this enrollment step. You do this by selecting an existing record link and editing the pre-populated information, if needed. You can also add and delete a record.

To add identifiers:

  1. Select the Add Identifiers hyperlink.

    The System displays the Provider Identifiers page.

  2. Select the Add button.

    The System displays the Add New Identifier page.

  3. Select an Identifier Type. (Required)
  4. Select an Identifier Value. (Required)
  5. Select a Start Date (Required) and an End Date.
  6. Select the OK button.

    The System saves the Identifier information and displays the Provider Identifiers page.

  7. Select the Close button.

    The System displays the Enroll Provider page.

To delete an identifier:

  1. From the Enroll Provider page, select the Add Identifiers hyperlink.

    The System displays the Provider Identifiers page.

  2. Select the checkbox beside the record you want to delete and select Delete.
